On March 23rd, 2005 at approximately 1:20pm an explosion occurred at the BP oil refinery in Texas City. The blast killed 15 people, injured hundreds more and devastated a community. Using extracts from the 2007 Baker Report, this programme examines what lead to the explosion and shows how decisions made at the highest levels, influences on culture, a normalisation of risk, and a lack of involvement of the workforce contributed to the disaster.
